Bad naps, reflux and EASY (looong post...)
« on: July 10, 2008, 17:55:01 pm »
Hi everyone

Can someone please help me...  :(

My DD is almost 8 weeks old. She has been on EASY since the beginning, but not very consistently.
She has reflux and she is bottle fed with AR formulas. I started a new formula this week as well as domperidona.
And this week things have gotten worst:

Her EASY routine has a few problems to start with:

because of her reflux, I can't change her diapers after eating... it has to be done before that, otherwise she will spit-up/vomit – the EASY became AESY. Also, she sometimes wakes up before eating and wanting to interact.
and
she was a really fast eater – 4 oz in 10 minutes. That's all the doctor “allows” her to eat and she really doesn't take any more...

Her AESY schedule was:

09:00 – feed
09:10 - “stands up” - the doctor told me I had to put her in a upright position because of the reflux problem (She gets cranky and cries in my lap after 20 min. I try to keep her in the chair but she cries after 5-10 minutes. Same thing in the crib)
09:30 – I try to get her to nap. I do it in my living room, TV on and soft daylight. If she is to agitated I swaddle her (she gets very hot with it sometimes and I have to take it off) or  take her to my room (no TV, only daylight). Try to do the 4S ritual.
11:30 – she is asleep (in the good days...)
12:00 – feed
this cycle repeats through out the day but she can sleep a little longer during the afternoon. When she stays with my mother or we go out she can do 3h naps. Usually naps vary between 45 to 1h30 to 2h.
6:00 – feed
6:30 – bath
7:00 – put her to sleep (the same thing as at 09:10...  :()
9:00 – feed
9:10 – stands up
9:30 – sleep ( she usually gets to sleep on her own)
 00:00 – dreamfeed
she may wake up at 03 and 06 am or only at 4 or 5 for a feed. She goes to sleep immediately after. I keep her on the upright position during the night and she sleeps in my lap.

Here is the problem now...

She likes a lot to smile and interact with people and the environment... she has been doing this more often and longer (which is natural) but she is also starting to get VERY cranky and do short naps.
Sometimes she would only take a few ounces and  then cries very loudly. This could go on during the feed, for 30 minutes...1 hour. Since I have started the new formula, this happens almost EVERY feed...
She is eating different amounts: 2 oz, 4 oz, 3 oz... and she rarely  asks for more than 4 oz. During all the formulas if have tested she kept on spitting and vomiting. Her crib has ~45º inclination.
She is refusing to go down for a nap, crying very hard and stretching her legs vigorously. She only sleeps 10 – 15 – 30 minutes naps. She wakes up frequently and I have to settle her down with paci and my hand over her legs/ shoulder (sush-pat really annoys her). I try singing, swaddling, letting her sleep in may lap... sometimes I take her to my bed and not even then she will sleep!
She sleeps weel at night.

Is she getting too much stimulation? Can the new formula cause these changes? Should I put her to sleep her naps in a dark room? (I would like her to settle in a “normal” environment)

Thanks for you help and sorry for the looong post....
I am feeling really sad and frustrated... I can't seem to understand her cues....  :'(

At least she sleeps at night!

My DS had reflux and I was never able to do EASY with him. (I did plenty of hard, though). He also didn't want his milk. TBH I never got it right until he grew out of the reflux by himself, and then all his problems went away.

I think with such poor naps she will be overtired, but the issue is whether she's JUST overtired/overstimulated, or also hungry or in pain from reflux. Tracey said you had to sort out the reflux, and then do EASY. I think now I could have pursued it more with my GP, but I did't know, until he just got better, whether his problems were reflux or AP because of reflux.

Sorry, that is probably not much help.

Thanks...!

At least I know that is not possible to do EASY while the reflux isn't controlled...  :).. I was really getting stressed with that...

Maybe then she will be able to do nice naps...

How was you baby's routine? Did you have any problems with Eat/Sleep/Activity? How often did she eat?

Meanwhile I went to the doctor and she advised me to do a 4h routine with her... she is very young but I will give it a try...

By the way... could you tell me what is TBH and GP...?  :-[

And again, many thanks!

TBH is 'to be honest' and GP is general practitioner, i.e. doctor.

Hi there and hugs to you, how long has she been on the meds and new milk? Some meds take up to 2 weeks to show a great effect, so may be a case of wait and see if they help, if they dont there are other meds you can try.  Hmmmmmm to the 4 hr feeding routine, tbh I would follow her hungry cues, if she cant go four hours, its going to make her hysterical and harder to feed maybe as reflux babies are fussy feeders anyway. I see where the doc may be coming from but the reality of actually doing it could be very stressful for you both. Reflux babes can be snackers as they feed little bits to soothe the acid burning their throat. Once you are certain the reflux is under control things "should" fall into place. Big hugs, its hard with reflux I know, Ive been there :(

Hi there, and big hugs!  Absolutely do not stress about getting your reflux babe on EASY.  My DD 2 has terrible reflux, still suffers with it but thankfully her medication keeps it under control (with the appropriate weight-related changes in dose), so I know how hard it can be.   Unfortunately, when our babies are so uncomfortable, there is nothing we can do to sleep-train them.  Whatever habits are formed will have to be broken later, but now you need to do whatever you can to keep her comfortable.

Is she on Zantac or anything besides domperidone?  It doesn't sound like her reflux is under control yet, so if it doesn't get better soon, I'd check with your doctor and see if she can have some more help that way.

Reflux babes take smaller, more frequent feeds because larger feeds make them feel worse.  I don't understand why your doctor would have told you to feed her every 4 hours, and if it were me, I'd question that.  My LO ate every 2 hours (BF so I didn't know the amounts).  She fussed through every feed, preferred to be upright and wouldn't sleep except on me.  Once the medication began to help, she slept way better, napped better, and let me sleep train her (I was her prop, so I used a modified pat/shh to train her to go down independently - I rested my hand on her chest instead of patting and then shh'd).

Have you elevated her crib head?  That often helps.  I also carried her in a sling for naps until she could be sleep trained.

That's all I can think of right now.  Have you checked out the Colic, Reflux, & Crying board under the Health section?  The moms there are full of great information and wonderful support.  If it makes you feel better, even though she's suffered so much with reflux, my DD2 is a happy, healthy, wonderful little girl who is on a great routine and has been ever since she got the proper treatment.

Hi there! Thank you so much!
A few things have changed since my last post...
I have my DD on another formula now (she started monday) and seems better... she's been on 1 mg domperidone 3 times a day, for 3 weeks now. The doctor came up with the idea of a 4 h routine because DD often spits-up "cheese-like milk" and she thinks that my DD didn't have digested completely the previous feed. So, 4 h would allow her to digest completely the milk. My doctor thinks my DD doesn't have reflux because she is gaining weight every week. Maybe I am getting the symptoms wrong...
Guess what... She can hold 4h without crying of hunger... which is a surprise for me. But I am doing a 3,5h routine.
Interesting thing about snacking, though. Sometimes she eats very little (2oz) and then, when I think she's crying to get to sleep (1 hour later), she actually wants the rest of the bottle. I don't want her to become a snacker...

What do you think about all this?!

Thanks!

My DD2 never spit up so she has what's called silent reflux.  Weight gain does not rule out reflux, and in fact some babes overeat because the milk soothes their throats, so they can be overweight.  Again, don't worry about your DD becoming a snacker... you have to let her lead in some ways, because she knows what makes her feel uncomfortable.  I wouldn't purposely feed her small amount, but let her be the judge of when she's done and when she needs more.  My DD still takes a night feed at 9 months and I believe she still takes smaller feeds during the day and therefore needs the feed at night to make up the calories.

I'm glad she's doing better on the formula.  I'd revisit the medication issue with your doctor if her symptoms don't resolve.

Gotta run keep me posted!

Ok, it seems that reflux babes do gain weight sometimes... you're right. I guess I'll have to relax and not think so much about EASY and let all the "accidental pareting" happen  ;).

My DD is doing an eco at friday and then I'll know for sure if she has reflux or not and adjust the meds with my doctor.

Thanks sooo much! Hugs :)

Hi!

Just to let you know...

My DD is on domperidone and the AR formula and she seems a lot better. She still eats very little during the day (she ate only 2 oz at her 3pm feed...) which affects a little the EASY routine.

I was trying to make her sleep the entire night, but if she isn't getting enough calories during the day she will wake at night for a feed. I do the dreamfeed at 11pm and she usually wakes at 3-4am. Than lasts until 7-8am. During the 3-4am feed I give her a little less - she drinks the hole bottle but still isn't very hungry in the morning...

I can't get her to eat more during the day...  :-\

Anyway, thanks for the advice!

Hugs!  I know, it's hard with these refluxers.  We're still doing a night feed here at over 10 months!  Trying to wean her of it as she seems less hungry during the day now, but also questioning how controlled her reflux is!  I think we just have to be patient with these bubs!  I'm glad your LO seems to be doing better!  :-*
