30 min naps

PLEASE HELP!!

I am new to this and would really appreciate any help from anyone!!

My 12 week old has starting waking up after 30 mins when I put him down for a nap (since sunday) Its everytime and on the dot!! he used to swing in and out of a 45min habit but i thought i cud sort it with wake to sleep (shh pat at 30 mins) is this the result???
I am really worried as it has def turned into a habit. I stay in his room shhhing till he is fast asleep (can take ages) but he still wakes up crying, I take him up as soon as he is tired. what am I doing wrong? when and how would you do wake to sleep for 30 mins??

I am scared  It will mess up his good night sleeping if this goes on, very down - am loosing any time I used to have to myself

Yesterday went something like this:

Average day since sunday

Feed at 7 (varies)
Activity = 45 mins
Sleep 30 mins (eventually) Try to shhshh pat until next feed. Charlie seems to be alert but won't make it until next feed
Feed at 10:30
Activity = watch for yawns (30 mins -tired by this point)
Sleep = 30 mins maybe another 30 with shhh pat if lucky
gap where I don't know what to do with him
Feed 2:00
Activity = he seems to be knackerd staight away - try to get him to play for a while
Sleep+ 30 mins same as before
Feed 5:30
Activity = some kind of secon wind kicks in and he seems ok for 20 mins or so
catnap (rarely)
Bath 6:30
feed at 7:00
Bed 7:30

Has been known to go through until 7 but usually 6:30ish (sometimes wakes around 3)
He seems even worse today :(

I think your first awake time is to short at this age you want to aim for around 1.5hrs and then the first short nap is throwing out the rest of the day. So you want to aim for something like this:

E 7am
A 1.5hrs including feed
S 8.30am
E 10.30am
A 1.5hrs including feed
S 12pm
E 2pm
A 1.5hrs including feed
S 4pm
E 5.30pm
A including bath
E 7
S 7.30pm

Thankyou soo much!! that makes complete sense! I guess thats why he wakes up after 30 mins ready play but flakes out before feed?

Just two naps and a catnap? guess he will be quite tired for a few days whilst i try this?

Will start again tommorow and let you know how it goes :)
