Help me get dd down to one feed

That is per E.A.S.Y not per day.  Dd is now 14 weeks old & we have several problems which I am working on one by one.  I am hoping that if I get her feeds sorted it might help with the 45 min naps & hopefully get her sleeping through the night.

The problem is that she has a 10 min feed twice per e.a.s.y so it is more e.a.e.a.s.y.  She had problems latching at birth & didn't latch until she was 3 weeks so, at the time to encourage supply I feed her several times in her awake time.  We managed to get it down to twice quite easily but I haven't been able to get it down anymore.  She feeds on one breast for about 7 mins & then 3 on the second & visa versa after about an hour.  I have her on a 3.5hr easy for a few days b/c I thought that we were having 45 min naps b/c she needed to move.  Yesterday & today she seems not to be too hungry for the first feed & more hungry for the second.  I don't know if I was right to adjust her routine as it has made no difference to the 45 min naps

Any advise?  I 'll have to pop back later to post routine as it's bath time now.

This is her typical E.A.S.Y routine:

7am E
7.10 A
8am E
8:10 A
8:45 S
10:30 E
10:40 A
11:20 E
11:30 A
12:15 S
2pm E
2:10 A
3pm E
3:10 A
3:45 S
5:30-6pm E
5:40/6:10 A
6:50 E
7pm A
7:30 S
11pm dreamfeed

She is having 45 min naps at each nap at the moment.  I go in and do pat/shush but it takes quite a while to get her back to sleep.  When she wakes 45mins into her 3:45 sleep she gets very very upset & it generally takes until 5:15 to get her back to sleep so I leave her sleep the extra half an hour.

She also wakes at least once a night between 2am & 3:30.  If she has a second feed it makes her fussy from about 5-7am.
