Hunger or Habit????

My almost 8 month old is still waking up 1-2 times per night.  Sometimes I can soothe her without feeding her (which is not often) and then other times the only way to stop her from crying is to give her the breast.  I'm not sure if this is hunger or habit...can anyone tell me how you can tell the difference?
I've gotten to the point where I don't know what else to do.  Neither me or my DH are fans of the CIO method and I've run out of ideas.

Here's her EASY routine:
E  7.00 - 7.30am Wake and Breast Feed
A  8.00 Play (was giving her fruit, but doesn't like any of them so we've stopped this week for the meantime)
S  9.30-10.00 NAP (on average sleeps for about an 1hr, but the sometimes can get 1.5-2 hrs
E  11.00 Wake and Breast Feed & Play
A  12.00-1pm Lunch (2 tbsps)
S  2.00-3pm NAP (1.0-1.5 hrs)
E  3.00-4.00 Breast Feed & Play
A  5-6pm Dinner (2 tbsps)
A  7-7.30 start Bath, Massage, book, breastfeed
S  7.30-8pm BED

Wakes up between 12am-1am for the first feed and then again between 4am-5am.  Oh, and sometimes she'll wake up really early like 6am or 6.30am I'll feed her and she'll go back to sleep until 8pm and not want to eat upon wake up so I feed her before she goes down for her first nap.
