It sounds like you need to help your LO learn how to sleep in her cot/bassinet. The links below should help some.
If you start to put her down before she is asleep and continue to sooth her with shush/pat (or adapted) she will begin to get accustomed to feeling the weight of her own body on the mattress. Waking up a short time after being put down (asleep) can be because she is in a light phase of sleep, feeling weightless in your arms, then she is put down and feels her body weight and because she is in a light phase of sleep she stirs and wakes wondering where mummy has gone and why things feel so different.
Initially putting her down really really drowsy and continuing to sooth until she is in deep sleep will help her become accustomed to the cot environment for sleeping and over time you can reduce how much you need to sooth in the cot (gradually weaning the shush/pat).
I do need to stress though that sleep training is not advised if the reflux is not under control. Reflux pain can easily wake a LO and you must take this into account. I'd also advise checking her dosage for her weight if this has not been done recently in case she needs a higher dose. It is possible to help a LO to sleep with reflux but you must be gentle and always keep in mind the reflux can flare up, you can't train a baby that is in pain to sleep independently.

Also record the WU (wake up time) both in the morning and from naps. Remember to count the A time from when LO wakes up (not from the E time). If a nap is short you do not need to E immediately on WU, you can stick with the 3hr E so that LO gets a good feed and is not encouraged to snack.
