LONG night wakings

Hi! 3rd baby to use Baby Whisperer info with and STILL can't get it right all the time! LOL!

Colin is 12 months old. His daily routine is not so great due to our inconsistent schedule. :(  He's kind of all over the place and I'm sure that's the main part of the problem. He sucks his fingers to sleep. His naps are 1 1/2 hr & 45 m. His bedtime routine is the same, but the time varies greatly and therefore, so does wakeup time. Seems like we've always just returned from a trip, he's sick, he was up at a crazy time and needs a little extra sleep, or teething. We still breastfeed, but not to sleep. Nurses once or twice a day as we are weaning, and then once at night. The problem is after he nurses at night (usually after 4-5 hrs of sleep), he wants to get up and play. He will cry and cry if left to self soothe. 15 mins is too long in my opinion, so we get him up and he plays for a couple of hours (yes! We get up in the middle of the night at least twice a week!!!!) and then he returns to bed for the night. PU/PD doesn't seem to do anything because he doesn't calm down when picked up. I use a toy or video to distract him if nursing him and putting him back in crib doesn't do the trick. I'm EXHAUSTED! I THINK* the solution is to wake him up earlier than his usual 9am, but I'm always SO tired I can't seem to drag myself in there to do it! Any other ideas besides waking him earlier?

The other problem I face is when he is doing well on a 'schedule', then something disrupts it and he goes right back to this nonsense. I.E. A nap in the car = not long enough and then that night he will have a crazy night of being up for hours, therefore, the next day so tired he sleeps late and we are back to square one.

HELP! Please don't criticize! You don't know my profession or 'excuses' of why it's difficult to settle into a regular routine....

Sorry I didn't add that I have had no success with dreamfeeds, therefore I stopped trying them.

Hugs xxx. We're dealing with poor nights here too so I know how exhausting it is. Two things jump out at me - do you think he's ready to drop down to one nap?  Have a look at these links.
