Confusion with number of feedings

I have a 3.5 month old who is on the 3 hours feeding schedule. I have recently extended one of her feedings to 4 hours to get her ready for the 4-hour schedule.

I do have a question though about the BW

In the BW, it mentions that with the 4-hour schedule that there are a total # of 6 feedings (including the DF) Where is the 6th feeding?
A 4-hour schedule would be something like this:

Eat at 7, 11, 3, 7 and DF. This is a total # of 5 feedings, where does the 6th come in.

Is she considering cluster feeding? and if so she mentions not to cluster feed after 8 weeks.. Can anyone shed some light on this for me?

Where did you read there was 6 feeds? 1st or 2nd book and what page?

I am looking at the second book on page 95 and 96. Its a chart called Feeding 101 under the section Bottle feeding, how much.

With regards to changing her to 4-hour EASY, do you know what page its on regarding the specific strategy?

Actually I have the plan for 3-4 EASY.
