Screaming before Sleep

We started 2 days ago to put our 7 week old daughter, Gemma, onto a more structured EASY routine.  She has been on an EAS routine since she was born, but until now she would always be rocked to sleep, and would not sleep very well, and would have a very sporadic sleeping times.
We have been following the Shhh-Pat techniquie to settle her, but it doesn't seem to be working very well.  We wait for tired signs, take her away from stimulations, and have at least 5 minutes down time before begining to swaddle her.  She shows all the signs of being ready to sleep, but aS soon as she is swaddled she begins screaming, often hysterically, and then continues to scream until she is exhausted and starts to sleep lightly.  This often lasts 10 - 15 minutes.
She also wakes from sleeps ocassionlly after 40 minutes or so, and then gets hysterical if I try and settle her, and often I try and settle her for 30 + minutes until she has exhausted herself and is starving, so we start another cycle which is doomed because she is too tired to eat and play.

It is very distressing and makes me want to quit because I really hate upsetting her like this multiple times a day. 

I would really appreciate advice as I would really really like her to sleep independently, but I would like it to be a happier proposition.

Welcome!!  For starters, can you post her routine for us?  Typical wake up times, feeding and activity times, that sort of thing.  Easier for us to spot a pattern.  Just off hand, try starting her winddown 10 minutes or so earlier.  A lot of times, when babe is overtired, they react this way.  At that age, mine could easily go from one yawn to overtired and howling in a matter of minutes.  Also, on the EASY forum is a quiz to help figure out what personality she is.  VERY helpful to know.   :)

Have you made the "Know Your Baby" quiz in the EASY section? If she is a grumpy baby she might not benefit from swaddling ???

Gemma has mostly been in a 3 hour feed cycle since birth, and during the day you can almost set your watch by her 3 hour hungry cry's.  Generally she is a really good eater, she was a big baby at birth (4.2 Kg) and have been steadily gaining weight since. Typically she is a fast eater and is generally done in 30 minutes.  Iam am fairly sure that her eating habits are pretty good.
Until this week her daytime sleep and activity cycles have been somewhat all over the place.  I think I am more than a little guilty of overstimulating Germma, on somedays she was awake and seemingly alert for up to 1.5 hours after a feed, and on otherdays she would scarcly open her eyes long enough to feed. Likewise her sleep patterns varied wildley, sometimes she would sleep for the full 2.5 hours after a feed, and other times should would sleep and wake for 30 - 40 mins before she was ready for food again. She would always go to sleep being cuddled, and would often wake up startled to be in her cot.
Now I have been trying to get her onto a more structured routine, where she seems to be getting (and benefitting) from a lot more sleep.  Typically she will eat for 30 minutes, then she is active (inc nappy change) for 20 -40 minutes (mood dependant) and then sleep for rest of the 3 hours.  She has woken a few times after 45 minutes or so, and I have not managed to get her back to sleep.
At night she always has a 5 - 6 hour sleep, and we have also managed to introduce the top up feeds and the dream feed. 

She does seem to resist the Swaddle, but unfortunately she is also a very active sleeper and frequently wakes herself up with her jerking;
